A patient shows reduced engagement and motivation. Before revealing the answer, decide what distinguishes apathy from demoralization.
Ask whether the reduced engagement is pervasive across every area of life, or specifically tied to one particular struggle or set of circumstances. Apathy is a more pervasive indifference across areas of life, a general lack of interest, motivation, and emotional engagement that extends broadly rather than being confined to one problem. Demoralization, by contrast, keeps discouragement closely tied to a specific struggle — the person remains engaged and concerned about other areas of life, but feels helpless and discouraged specifically in relation to the particular difficulty they face. The breadth of the disengagement, general versus circumscribed, is the key distinguishing feature.
